Ryan’s Place is a specialized primary care practice that delivers compassionate, patient-centered healthcare for individuals with physical disabilities. We are a community-based practice that serves the Baltimore-Washington DC region.
- Primary Care Services: full range of primary care services, including preventive screenings, chronic care management, and personalized health education, all designed specifically for individuals with disabilities.
- Seamless Care Continuum: Continuity of care for individuals with disabilities. This includes coordinating care between specialists, pediatric providers, and adult care teams, tracking patient health histories, managing complex care plans, and scheduling regular follow-up visits to ensure no gaps in care, particularly as patients transition from pediatric to adult healthcare services.
- Telehealth for Primary Care: telehealth services to make healthcare accessible for individuals with disabilities and reduce the burden of travel, ensuring continuous care.
- Health Education and Patient Empowerment: programs to empower patients and caregivers. These initiatives provide information on managing chronic conditions, navigating the healthcare system, and advocating for necessary resources, giving individuals the tools to actively manage their health and care.
- Clinical Training Site for Disability Care: clinical site for nursing students and nurse practitioners, providing hands-on experience in disability-inclusive care. Trainees may work alongside experienced providers to learn how to deliver primary care to individuals with disabilities, gain essential skills in managing complex medical needs and enhance the quality of care for this underserved population.
