Transition to Adult Care

As teens with disabilities grow into young adults, their healthcare needs change. We guide families through a step-by-step, age-based transition pathway, providing the right support at the right time from early adolescence through adulthood. Our team works with you to plan ahead, set independence goals, and connect with the right specialists, therapists, and community supports. Together, we build skills and confidence, so every young adult feels prepared, supported, and ready for the next stage of life.

For families, transition to adulthood brings uncertainty. Coordinated care brings confidence, continuity, and hope for what’s to come.

For young people with complex needs, a guided transition to adult care helps protect their health, dignity, and opportunities.
